Yes!!! 2 episode to go! Kinda sad, but we all wanna know what's going on right? I was supposed to do this over the weekend but I got distracted... thanks to the everywhere is sold out to pre-order DVD saga (you know which DVD it is). Only 1 boyband will crash cdjapan site every time they release something. My brother tried to order something around the same time and was confused on why cdjapan suddenly not working? Something wrong? Now he knows why LOL... and also because I marathon-ed 'Dinner.' It's been awhile I saw food drama which are not... flat *hint Nobunaga no Chef*.

OK, so what do we have in 9th Episode. All mysteries starts to unveil but... still cryptic for lots of parts and especially without English subs... it's worse! I realise something... I thought Epi 6 was the worse, then I thought Epi 7 was even worse, and Epi 8 and now... Epi 9 has even bigger words than those previous episodes! What's wrong this drama? It's as if it has a mission to make me learn all those medical words that I'll definitely forget after this drama is done!

Since we've run through all the doctors' past, starting from this episode, the first scene has nothing to do with the topic of the day.
So, cute doctor is trying to find more info around her dad's research through Komaki sensei based on daddy's memo. She wanted to verify some details of the memo. Komaki sensei told her that he's seen this before. It's something to do with genetic engineering (I don't understand the rest of it), and it's from the Saito couple's research but seems that their research points on the analytical process were not clear then (?). Why is that? Because the accidental product was born... (me: masaka....). Then, Komaki sensei stopped explaining though. And Tachibana sensei has 1 more thing to say, and it's regarding what Komaki sensei is currently doing and it's related to his deceased son.

Bitchy reporter shows up again! And turn out Tachibana sensei is the reason of her dad's incident? And it's an unforgettable incident for the bitchy reporter... oh DRAMA!!!

So we have a new patient, a VIP, Machida Electronics' president himself. Pretty doctor knows his news well. Apparently everybody read the president's book and Tachibana sensei as well, much to pretty doctor's amusement.

Product placement! So, the president has pancreatic cancer. The tumor is a difficult one. So basically pretty doctor is saying that CT scan normally doesn't catch this type of tumor until it's too late, which is quite true from what I know as well (now you know how I try to link the words together hahaha) and the chance of survival after 5 years (or more than... omg, subs where are you???) is low.
So, basically the team task is to create miracle on how to fix this cancer.
And the best thing, the patient itself only know he's brought into the hospital because they found a polyp.

So, Takagi sensei's scouting session was done in the US. Apparently Takagi's Sendai accident case is cleared by the ethics committee (?) Takagi sensei know what he's doing during the accident and instead he got a 'thanks' from the patient. He kept on playing the 'what if' scenarios but couldn't find the answer. And head center launched his recruiting punch line: why not go back to Japan and find out?

And they met the patient. Takagi still managed to flirt in a split second with Tachibana sensei scoffed on him LOL.
The president seems to be a nice person and as told before, he only knows about the polyp and..

The son was obviously concern. OK more info, the tumor is 4cm big. Takagi basically explained that the operation can be difficult and cause bleeding, which is the same as the last doctors' explanation. So, to add on problem, it could lead to pancreatic diabetic (such word exists!). Phewww, that's the main point of it.
Son insisting the doctors to not give up on daddy and the last hope line goes to... Takagi sensei!!! LOL If the look can kill, look at the way pretty doctor glare at Takagi sensei after he stole the line haha...

Last hope pleading over the phone (look back previous epi) but the doctor didn't know what it is. So, there's another small boy, with big stomach. But Saito couple only had 1 kid. Well, there's a lot of rumor that it's not a kid obtained morally (me: masaka...) and there's no clarity on that because Saito Hitomi died. One small detail was that the head center hummed during difficult operation.... er.... sounds familiar?

Not gonna explain this scene, too many words... head center just want to make sure that his team creates miracle...
So as usual they're discussing the treatment plan. They have come up with 2 plans, the protecting and attacking plan (something like that)... Takagi is recommending to do blood vessel baypass considering there's chance of bleeding then quickly do the surgery... but the risk is super huge. Tachibana sensei only cared whether it can't be done. Hatano sensei raised his hope up but Takagi waver and everybody were disappointed, ha! Another problem, it's the VIP they're operating on, no kidding! The pressure!

3pm and pretty doctor naturally bring the dessert and we have the dessert eating scene back! this time it's mango pudding (the dessert looked a bit plain to my disappointment, but pretty doctor eating it certainly doesn't look plain haha).
So the discussion goes back to iPS treatment (look back to couple episodes back).
This time we're back to epi 1 where Hatano got his inspiration from the dessert he ate, and he managed to annoyed Takagi. So the main point is that 'take out and put back in.' It's like self internal organ transplant where they take out the piece of the pancreas, clean it from the tumor and put back? Make any sense? Found the word for it: Autologous transplantation.
OK, the rest of it... wait for subs... spent 2 hours already and it's just 20 minutes of the episode...

Confrontation! So, Soejima let Takagi know that he knew about Takagi's history. Seojima was basically telling Takagi don't blame too much on himself. Whether her death was right or wrong, is not the interest. The possible treatment medicent was available abroad but still under approval process in Japan. Takagi argue that the outcome maybe the same (?) but Soejima emphasized that the choice was not there in the first place.  Wow, Soejima is deep. He has the vision and he already had the plan.

Hi! Hi! Back again ... and thelinks are up for ep 9 ... so I took quick watch of the ep, and am only halfway through your caps ...

Just a few things ... both type 1 and type 2 diabetes is usually controlled via use of insulin. But with pancreatic diabetes (which pancreatic diabetes mellitus) although a type 2 diabetes, it has the added complication that insulin is not a good thing. (too complicated for my brain to comprehend let alone to explain so sorry) So it is strictly controlled by watching food intake and strict blood glucose monitoring. And although intensive insulin therapy is used ... a very conservative approach is taken. ie, is highly monitored and very strictly administered.

The thing with pancreas transplantation is you have to leave the original one in and attach the healthy/donor part to it. Although they do procedures for diabetic patients, not so for the case for pancreatic cancers. Not just because the risk is too high but there is a huge huge chance of the cancer coming back.

ARDS - Acute respiratory distress syndrome
ALI - Acute Lung Injury
EPC - Endothelial Progenitor Cells

As far as I can find out autologous transplantation of EPC is a novel therapy in preventing ARDS/ALI for patients who have undergone vascular surgery, hence the use of ECMO machine (which oxygenates blood as surgery is being done).

It could be that the scriptwriters have used a little poetic/artistic license here so they may insert other conditions and procedures to fit the story line??? *shrugs* I dunno,

As all the reading I found says that if you have pancreatic cancer .... SURGERy IS Not AN OPTION. Any pancreatic surgery usually is to fix things like the diabetes problem. Surgery takes about 6~8 hours and recovery about 3 weeks stay in hospital.

This might be another patient they cannot save. We'll see what the scriptwriters give us eh??

I think this drama, if you have no knowledge on healthcare treatment whatsoever, it'll be very difficult to digest all those medical terms... I know I'm a bit lost even with the English subs with those that have subs already...

Very interesting that they're suggesting operation where it's not the option in reality... I think Last Hope scriptwriter has enough detail aka consulted with the real medical field people to be able to create these scenes... and yeah, excited for Epi 10!!!

... and I'm still in the office, *cry* 1 hour to midnight!

Unfortunately ... although I'm not in the healthcare industry ... I have parents that are constantly in hospital for one reason or another. (My Dad is 80 and my mother is 78 so it's all age related stuff) Still you get to learn stuff, standard procedures, meeting up with doctors and consultants, follow ups. What the job scope of nurses are, how far interns are allowed to practice on patients, among other things.

I've also had the unfortunate experience of having to go into surgery three times ... and hopefully never again.

I must admit that I am impressed with the props the drama has been allowed to use. Because as far as I can tell ... everything is the real deal.

The equipment(the operating theatre, the equipment that Soejima used to do the microsurgery ... I just coincidentally saw machines exactly like it on a National Geographic documentary - and the surgeon doing the surgery right down to the fact that the equipment too was wrapped in plastic!), the gurneys, the 'teaching hospital/research hospital' environment.

It's completely awesome. I am truly impressed.

If I did have any quibbles at all ... it would be the set up of the meeting room. It feels just that little bit staged. Although I suppose they had to design it where we could see them all at their 'roundtable' discussions at their ... uhmmm round table.

These are the knights (in white coats) of the roundtable giving their patients the last hope.

I was thinking that one of the reasons that the pretty doctor was aghast at Takagi saying the last hope line is probably because that ... maybe he knows that there IS no hope ... not in this case anyway.

I agree with you that it does show that they do have experts on hand here to advise on the medical aspects of the drama.

True, spending time in hospital will give you lots of info that you never thought off... I still remember that in the first 5 minutes in the waiting room, I already know that belacan can cause cancer, and cancer can be everywhere... even on your tongue... hieeeee....

I agree with you on the props coz what they use in the OP scenes are not much different from what we see on the real OP scene from eg. youtube etc (all those demo vids)...

Also agree with you on the conference room... too nice and grand for a university hospital LOL and don't think real life have that luxury to have discussion over a patient in such manner LOL... and the round table thingy... LOL, I never thought of that...

Interesting view on why pretty doctor glare at Takagi over the last hope line, but really, it looks like he doesn't like people to steal his line... something like that... Hatano Takumi is an interesting character...

Just thinking aloud here ... maybe Tachibana's dad, Komaki and Saito were all workiing on similar research. Some overlap somewhere, as most researcher do rely on others' work to guide them or give them hints. So they may have known the 'big picture' but ... sworn to secrecy ... or something.

Also in SG ... if you are about to undergo complicated surgery ... you do get shown a pretty picture, if you want it photocopied ... they'll give you a copy. (I know they do so at NUH and Mt Elizabeth. Personal experience so yeah ... ooh mysterious again). They talk you through the procedure you will undergo. You do have to sign a disclaimer form ... that you have been explained all the risks bla bla bla, so your family can't go sue them is something goes wrong (ie if you die, become paralysed or whatnot). In SG too you get to meet the surgeons as well as the anesthiologist that will do the op. For transplants ... usually a big team so you only ever get to meet one to three of the suregeons. Of course your attending consultant in charge.

Oh and so sorry for bouncing my ideas at you ... I hope you don't mind.

The drama definitely select the cast well for this drama, to please our eyes, definitely hahahaa

Oh yeah, I got the same feeling that Komaki and Saito has something going on with their research and it's as if they're doing the same thing and Komaki is just sort of developing what they have before...

OK, so maybe from my experience, the procedures done on my dad (heart cateter) in indonesia and my mom (brain biopsy, which was done at Mt E and failed horribly T__T) didn't have the doctor giving me nice explanations in graphics over the operations. They did make my parents sign TONS of indemnity shitty papers... and now my dad got traumatized into going to Mt E for treatment... I think he swear to himself never to step a foot on Mt E anymore... hell, even I was traumatized...

Oh well, dramaaa... Epi 10...

Thankfully the op. that I went through didn't traumatize me too badly although appparently I took too long to to come out from the effects of the anaesthesia that I caught bronchial pneumonia ... and ended up in ICU for three days, before I was safe from germs as my immune system was quite compromised.

Yeah this was at Mt. E too!

Even though I will be glad to see the mysteries all revealed, I will be sad that there will be no more Cute and Pretty Doctors! ^_^

Yeah! the drama will be over soon, cute and pretty doctor will be no more soon... I hope this drama will become a platform for Aiba's resume in getting more BETTER written drama offer. He can act, even though he's no Nino (come on, just be real), but he's definitely better than some people too... :D :D *cheering for Aibaby* and producers need to realize, he'll look good n pretty in any roles that he'll be in LOL...
