Time sure flies so fast and we're only 1 more episode to the full reveal! And after that no more pretty and cute doctors to entertain us... T__T *somehow I feel lonely muahahaha*... and most likely our pretty doctor will change his black hair as soon as the drama ends... more T__T. Though he'll rock any kinds of hair styles (given his track record), but I like his black hair A LOT this time hahaha...

Put that aside pretty doctor's hair... let's see... Epi 10 has the lowest rating so far, and I think I know why... It's not because it's a bad drama, seriously, if it's that bad, I won't be making caps for all episodes and look forward for the next epi every week. The fault in this drama is that I think the drama team forget about the audience this time. They're too engross with all the details, I mean, have you ever watch japanese drama in this detail? Even Dr. X was not this complicated and that's why Dr. X's rating was sky high! Starting from episode 6, you'll see more and more complicated medical terms and as you reach to Epi 9, you're flooded by medical terms that you don't really hear in your daily life. Normal people will understand heart attack, anemia, leukemia, by-pass operation. But normal people will not understand glioblastoma (epi 6), myelodysplastic syndrome (epi 7), pancreatitis (epi 9) and many more. I can imagine the confusion and chaos in audiences' brain trying to digest these terms when they're watching this drama on TV, live. Heck, I still have to wiki those terms (and read those wiki more than once) when I watched this drama with English subs (and we better be really really nice to the subbers because they work extra hard for this drama). And FujiTV releases the glossary terms on the website after the drama ends... is it a good move? Who would remember those terms after they watch it. My other guess is that the rating is low because everybody is recording it in their HDD. Ha!

OK, so, Epi 10 is basically continue on from what we left off in Epi 9, so if the audience skip Epi 9, the hell they're going to understand what's going on in Epi 10, thus... the rating... but pretty doctor is really pretty in this episode LOL... as always...

So, Machida president condition is now on the lungs. the lungs' pulmonary alveoli are hardening and the respitory tract's internal pressure is not high, so he can't really breath (something like that... I think)...  thus need artificial respitory administer.
K, they can't use steroid, positive (?) ventilation... so they end up discussing... lungs transplant.The brain death lung transplantation in Japan for both lungs, only applicable for patients below 55 years old. Patient is 60 years old, so option is out.
In the midst of the debate, Soejima mentioned ECMO and here's the wiki... I feel that I have to wiki all those terms else I have no idea what the heck they're talking about... so imagine those people watching this drama live... it'll be very tiring to be honest hahaha...

So, here comes another option, living body lung transplant.... i think the rest i'll leave it to subs... they mention something like if they do it, 70% of the lung function won't be back. And speaking of donor, they need 2 instead of 1 because with just 1 donor, the risk is too big.

So, now the question is who'll be the 2 donor? We're left with the brothers who are not getting along at all. One will do anything for daddy, the other one couldn't care less if daddy dies. And even with that, there're risk eg. intercostal neuralgia and out of the cases they've known, none of the patient survived. Then there's also a risk if the body rejects the transplant, and in Machida's case, change of survival is 50% (I have no idea what's the 5 year means though, is it chance of survival to live the next 5 years?).
And pretty doctor's dessert this time is caramelized doughnut. It looks yummy and pretty doctor eat it as if it's... a bagel LOL...

Hello, I've commented here before, haven't I? I mean on one of your Last Hope screencaps posts :P
I've been following your posts for awhile, and I thank you for all the time and effort that you're taking for sharing this with us.

btw, let's just say that I've watched the final episode, and I gotta warn you that you might want to prepare some tissues, coz there won't be a dry eye in the house. Or at least that's what happen for me. Even though I only understand like 40-50% of the drama (because believe me, I only learn Japanese through Arashi's shows and a basic Japanese lesson that I learn for about a year and it was 10 years ago), but I think I got the big picture. I don't really like the ending. It was good, but something just doesn't sit right with me. And I've rant about it on my FB :P

It might be too late, but I should say that I'm an MD (used to practice as a GP but now I'm more of an educational staff at med school), so I kinda know what they're talking about, when they speak in English terms XD. I should say that the writers have done excellent job in researching and quite detail in execution. Their execution of physical examination is good and surgery scenes looked so real. The cases are quite difficult, and I have to admit that I haven't met some of them, since some are more of specialist's competence, not a GP's. Takumi's GP is kinda a special case, since he was also trained as surgeon. I'm not like that haha.

Anyway, I think this will be the end of my rambling. Looking forward to your screencaps of the final episode, and would like to know your opinion on that.

i watched other j-medical dramas too (not all, but some) n i find last hope has the best props n details... its so details that i think this strong point kinda back fired a bit at some point hehe... but i still like this drama regardless coz the casting is good, story is interesting, n most of all, pretty doctor hahaaaa
wah, need tissue? i read yamakaze tweets n i was like... noooooo, but even she being native japanese was confused over the drama,all the more we should take our tme to watch so that we can appreciate more...
i do notice that those who work in healthcare can better appreciate this drama for all those details, not like me having to wiki while watching, and even that sometmes i still have no idea haaaa

I don't know if you like spoilers or not, but I just gotta say that I think the writers for Last Hope were inspired by the movie "My Sister's Keeper" (there's a wiki for that :P) for Takumi's situation. Only I prefer the ending of that movie better, than Last Hope.
For Tachibana/Aitani situation, I heard a term called "Chimera", which is a unique situation where a person can have two different set of DNA, due to an incomplete twin-formation. I think you can wiki that also XDD and I actually learn this from the early seasons of CSI Vegas lol. Then I've some light reading on the subject on my own.
I still need big help to understand Soejima-Komaki's situation and Ogiwara's case.
and I keep stalking DA for the HQ/Japanese-Chinese subbed version of this last episode lol

I'm a fan of Grissom too!!! I love that guy. The new season of CSI Vegas is kinda getting interesting though. They have new boss, D.B. Russel (acted by Ted Danson, since season 12) and his character is kinda interesting. You might want to give it a try :P

Heee ... I'm back ... ^___^ , as usual ... but again only half way. it helps me break it down better this way.

With the transplantation thing ... for something like the lungs, not only a blood match is required but as close to a tissue match is highly desirable. It lowers greatly the risk if rejection.

So problem (1) I don't think that the wife is a good candidate unless her blood type is the same. The grandson is probable because of the blood ties.

As with all transplants, the risk to the donor is also high. ESPECIALLY with the lung because (also like the kidney donation) you need the whol organ. Because ... it's like a balloon yes ... you have to have the whole thing intact otherwise it wouldn't work.

With lungs there is an extensive medical assessment required. Fitness, health and age of donor. Weight is also an important consideration. The lung size needs to be able to fit in the lung cavity of the recipient.

Also it does take a while to run the tests as well. X-rays, blood tests. Unless of course it is a fast track case ... they also need to ascertain that the recipient will survive this very high risk surgery. So even though the recipient might have a diseased lung, they need to make sure that they don't have other complications. (In daddy's case, he already has compromised blood and heart works, other things like kidney function too)

They also need to look at the life of the donor. (Yes seriously) For example in really young people they asses the risk to them. They also look at how many dependencies they may have and the type of work they do. With a kidney donation, life for the donor can pretty go back to normal. But with a lung. ... They will make you sign papers that say you know and understand the risks, and that means that any dependents you may have cannot ask the same of you in the future.

On the personal front, I do question the ethics of asking a parent to give up something for an already sick parent, when he has a young son that may need his vital organs in the future. (But that's just me, and because I am a parent - maybe) Also if it had been me, and say my daughter has a child, I don't think I would ask of her such a thing. (but then again that's also just me)

And yes ... I noticed how he sits ... so perfect I adore it to pieces. I took piano and my teacher was quite strict, and she would place a ruler on my back so I could align my back to it. Although she was quite the dragon and very strict, To this day I have good posture, and I will forever be grateful to her. SO many people these days sit so incorrectly. I wonder how they can breathe right. Heheh!

The Japanese have the knack to look forever young! It's unfair. ^__^ But it's ok we can have pretty ikemen to look at and drool over! Wheee!

Ahhh see ...The daddy has seen sense ... he has had a good run at life. It would be wrong for him to ask such a thing from his son. ^_^

So the scriptwriters also see life as real people do ... I like this a lot. I think this is one of the reasons ... why I do like this drama. A lot of it resonates with me as true as well.

Maybe the scriptwriters as well as using real case files and having experts advice, they might have interviewed real patients too. A lot of the situations feel that they have been thought through well as well as giving thought to the outcome,

I do agree with you that they got a little bogged down with their attention to detail. But then you see they have fans like us that will check up on stuff. On a personal front I like that they have made effort with the attention to detail, because it does feel like it holds the story together better.

Agree with you, the scriptwriters for this drama have brain! They did their research, for sure. Even Aiba-chan did his homework for this drama, I read somewhere that he did go to the hospital and shadow a doctor do his round or something like that...

We're lucky because we don't need to watch this drama life and we have the 'pause' button haaaa... I really do appreciate all the details because it makes this drama look so natural...

I want the recipe for that caramelized doughnut, it really looks good! T__T maybe I should make the chocolate cake again... hmmm...

thanks a tons for your explanation, that explains a lot why Shinichi hesitated so much to donate his lungs, I know it's risky but with that kind of risk? OMG, the little brother there is really... blinded...

on your ethics question, you get your answer at the end of the episode ha! I think if I were a parent, I won't ask my children to donate something that important that will affect their entire life, let alone if my children already has their family to take care off...

ha, i'm one of them who slouches, and i got scoliosis too to make it worse... my mom was not strict enough to make me sit straight, and the way aiba-chan sits... OMG, it's like tea ceremony style... maybe grandpa johnny put him on some lessons to prepare him to be a super idol hahaha...

It's easy to forgive the little brother though. Because your world view changes once they put the baby in your arms. And if he isn't a parent. It is hard to see their point of view.

LOL, Miyake Ken is a vampire in disguise LOL, he makes edward cullen look haggard, for comparison sake hahaha... but really i don't believe them saying that they don't do anything to their face and skincare, definitely lots of company sponsored facial treatment scheduled in their routine by their managers, else, how can the 40 years old nagano look the same since he's in early 30s? hahahaha...

True, your view will only change when you have your own kid. That's what my dad always told me... parenting change your perception in viewing things... I don't have a chance yet, but I hope i'll experience it in the future hahaha...
